Student Race/Ethnic Survey

The Illinois State Board of Education is requiring school districts to re-identify race and ethnicity for all students beginning next school year. Students will be bringing this form home in March with an explanation on how to complete said form. The form is to be returned to the student's school by March 29, 2010.
Please look for this form the first week of March, and return it to the school as soon as possible. Thank you for your cooperation in providing us with the needed documentation.
